For clients receiving services under Ohio Medicaid Home and Community-Based Services (HCBS) waiver programs, information may be shared with:

• Your managed care plan (if applicable)
• Your authorized case manager or service coordinator
• Quality assurance and oversight agencies
• Other providers involved in your care (with your consent or as required by law)

These disclosures are made as required for service authorization, billing, quality assurance, audits, and regulatory compliance.
